You don't need to do that every time - rinse our bottles well after pouring a beer, and sanitize before filling with starsan is sufficient. Bleach is a way to recover your bottles following contamination - but don't use it with plastic bottles (or any plastic equipment), and rinse very, very well afterwards.

As for what causes the staining, I don't know, and I don't think I want too....
Yesterday, I filled a stained bottle with pure Javex and let it sit until today, perfectly clean
Don't use a sugar solution for priming. This in itself can lead to overcarbonation. Dissolve about 60g of dried malt extract in boiling water, and boil for 10 minutes. Use this solution for priming. The beer will taste much better.
